We have integrated with two great services that allow you to create custom forms for your site: Wufoo and Form Assembly. Both services offer free and paid plans, so you can select the one that is right for you.
When you drop a form widget on your page, a dialogue box will open up. Click on the link in the bottom right hand corner and you will be taken to the form service. Sign up for an account and create your form. When you are done click "get code." Copy the code and paste it into the SynthaSite dialogue box.
Please take a look and tell me if one of these options meet your needs, or let me know if you had something different in mind.

Inappropriate?I'd like to give my site an e-mail subscription form.
I've looked at Form Assembly and Wufoo, but I can't find that part. -
Inappropriate?Here are the steps for Wufoo:
1. Log in or sign into an account.
2. Go to create a new form or you can go use of one their pre-defined templates
3. If you're making your own form, then you drag the items you want from the left hand side to the right and drop where you want them.
4. Once done, click on save form.
5. You can set up your email notifications so that you receive the emails when someone subscribes - save it.
6. When you're finished go back to "forms".
7. Click on "code" for the form you just made and then paste this code into your HTML widget on your site.

From what I've seen, at least in Wufoo, the forms allow people to send me their feedback. How do I make a form for people to be notified (by e-mail or RSS) every time I put something new on the blog? -

People can choose to subscribe to your blog by clicking on the "Subscribe to Blog" button on your blog.
You can integrate newsletter registration forms into Yola with Aweber.com http://www.aweber.com/ and the HTML widget (add the widget to your page and paste the Aweber embed code into it).
This service does cost a fee, if you would like to use a free service, you can try mailchimp http://www.mailchimp.com/ or autoresponders4all.com http://www.autoresponders4all.com/.
