Good widget for creating a form
I like it so far and it's free if you only need one form:

This is a reasonably good form creator and although they say there's no HTML to get involved with it still allows you the undisclosed option of modifying the HTML. This is a good thing.
It has a solid basic functionality. It's probably one of the more basic form types.

Inappropriate?thank you Jonothan, I was getting tired of looking for something free, lol!!! Why is it every little thing has to be so hard, I am losing interest so fast in doing this dang website, but i've been trying for several months so I guess I am determined to make it fly, lol

Inappropriate?sure thing. I find that good widgets are hard to find. Yola has found some nice ones. In general though, imho, Widget websites don't do a great job at categorizing or showing which ones are really useful/complete.

Google docs has free forms also....the info goes right to a spread sheet that you can keep private or share. -
Inappropriate?I recently stumbled across http://creator.zoho.com/. It's free and you can build whatever you want! You can also control what happens when a user clicks on the "submit" button. Sometimes form sites redirect you to their site to process the click but this one will allow you to direct the user wherever you want to go. It also has website integration functionality!
