You can take a look at some options available through the Widgetbox Gallery.
Another alternative is to have a look at www.js-kit.com. They offer free and customizable comment box.
You can also look at free guestbooks which may offer you similar formatting as a comment box.
Copy the HTML code into your HTML box.
Please note that with the JS-Kit, make sure you're the first one to leave a comment as this will give you your administrative control.
