Error on Form Submit

Can someone help? I am getting a 405 error every time I submit my form I created. I used javascript and html to make the form (so it would add price automatically) then I used php to send the form by email (an option I found online). Can someone maybe help me fix this problem or suggest something else. I need to send the form results by email but I don't want to use mailto because I don't want the costumer to have to send it. Try the form on the link (you don't have to enter all info) and see what you get or maybe if you can help me fix it. It would be greatly appreciated. Thank You!
