The forms on my site aren't working!! When anyone tries to send in a form, it's sending them to the Yola home page!! What is going on???

Yes, please help. Good thing I tested this before launching. This is a promotional sign-up and I NEED to receive those submissions...

Customer sent me a message on my Yola form and i didn't get it! Thank god he added me on skype after.
To my knowledge it just started today. It was working fine last night when I was testing stuff, and customers were sending in the information I was asking for.
