Need a little HTML code help please for Contact Form

Hello again, im doing this html form for people to contact me for puppy requests, (see screenshot) but i have no clue what to write for when they hit the submit key i recieve the form in email. any help please?

Hello,
I don't see a field for where you can input your email address for the form to come to; possibly this is something you set up in your account with the company you got the code from.

Are you familiar with the forms available to you in Synthasite from WuFoo and Form Assembly? They are really easy to build and manage.

I will give some instructions just in case you haven't seen them and would like to give it a try:

In order to put a form on your site click on the widget toolbox at the righthand side of the screen, and then click on the "Forms" folder. We have integrated with two great services that allow you to create custom forms for your site: Wufoo and Form Assembly. Both services offer free and paid plans, so you can select the one that is right for you.

These are the steps for adding a form:
1. When you drop a form widget on your page, a dialogue box will open up. Click on the link in the bottom right hand corner and you will be taken to the form service.
2. Sign up for an account and create your form.
3. When you are done click "Get Code."
4. Copy the HTML code (use Ctrl-C or highlight with cursor > right-click > copy) and paste it into the SynthaSite dialogue box that opened when you dropped the widget (use Ctrl-V or right-click > paste to paste the code)

That's it, you will have your own custom form on your site and your responses will be managed in your Wufoo or Form Assembly account.

You can edit your settings on Wufoo or Form Assembly so that the information completed on your form is emailed to you, or you can visit your account to get the information on any forms completed.

did the form assembly one, and when I hit submit screen just turned white and stayed like that

Ok found where to add the redicert link after people hit submit but its not getting me there, any help pls?

The Wufoo plan is free up to a certain number of form submissions per month. You should check it out, really easy to use and the forms look really nice!
