Simple form in HTML with Clear and Confirmation

There are often requests for help with Forms on this Forum. Although there is a Form widget provided by Yola which is enough for most users, there isn't a Clear (restart form) or Confirm Request (button for visitor to ask for a confirmation from you), which can both be useful.
This code provides a simple form that has these elements included, and is all HTML so is easily adapted.

Don't leave a space between your email address and the query in the first line.
