There are often requests for help with Forms on this Forum. Although there is a Form widget provided by Yola which is enough for most users, there isn't a Clear (restart form) or Confirm Request (button for visitor to ask for a confirmation from you), which can both be useful.
This code provides a simple form that has these elements included, and is all HTML so is easily adapted.

<center>
<big>
<form action="mailto:your own email address?subject=what you want the subject line to say" method="post" enctype="text/plain">

<table width="500"border="5">

<tr style="background-color:#bbddaa;">
<th colspan="4" style="font-size:20px;">Form heading</th>
</tr>

<tr style="text-align:center;">
<td colspan=2 width="250" style="background-color:#bbddaa;"><b>Your Name</b><br /><input type="text" name="Name" size="40" style="font-size:16px;"/></td>
<td colspan=2 width="250" style="background-color:#bbddaa;"><b>Your E-mail</b><br /><input type="text" name="E-Mail" size="40" style="font-size:16px;"/></td>
</tr>

<tr>
<td colspan="4"><textarea name="Message" value="Message:" rows="10" cols="85" placeholder="Your message."></textarea></td>
</tr>

<tr style="text-align:center;">
<td style="background-color:#ff3300;"><input type="reset" value="Clear Form" style="font-size:16px;"/></td>
<td colspan=2 style="background-color:#668888;">Confirm email?<input type="checkbox" value="Confirm" name="Confirm" style="font-size:16px;"/></td>
<td style="background-color:#bbddaa;"><input type="submit" value="Send Form" style="font-size:16px;"/></td>
</tr>

</table>
</form>
</big>
</center>

Don't leave a space between your email address and the query in the first line.
